Confidentiality
In a standard diagnostic evaluation, all information obtained during the interview and testing process remains confidential, and is shared only with the referring clinician. If collateral information is believed necessary to complete the diagnostic evaluation, individuals will receive assistance in completing a Release of Information form. When an individual is referred for a forensic evaluation (i.e., by a court or other agency in the context of a criminal or civil matter), the traditional doctor-patient relationship does not apply, and the individual is not the holder of privilege. Individuals are apprised of this important difference in confidentiality at the outset of the evaluation, and are informed of the specific purpose of the evaluation. Our practice is compliant with all relevant HIPAA regulations.

Appointments and Payment Information
If you would like more detailed information regarding the Neuropsychology Service, or would like to schedule an evaluation, please contact Dr. Lisa Sworowski, Associate Director of Neuropsychology, at 312.212.1776, x204. The Neuropsychology Service accepts most insurance coverages, including Medicare. Neuropsychologists typically invest 8-12 hours in neuropsychological evaluations.


 

 

 

Neuropsychology Staff

Diana S. Goldstein, Ph.D.
obtained her doctorate in clinical psychology from the Chicago Medical School, and completed fellowship training in neuropsychology at the University of Chicago. She is President and CEO and Director of Neuropsychology at Michigan Avenue Neuropsychologists, and holds a faculty appointment at the Rosalind Franklin University of Medicine and Science (formerly the University of Health Sciences/Chicago Medical School). Specialty areas of research and practice include traumatic brain injury, dementia, multiple sclerosis and malingering.

Lisa A. Sworowski, Ph.D.
obtained her doctorate in clinical psychology from the University of Kansas, and completed fellowship training in neuropsychology at the University of Chicago. She is the Associate Director of Neuropsychology at Michigan Avenue Neuropsychologists and holds a faculty appointment at the University of Chicago. Specialty areas of research and practice include traumatic brain injury, multiple sclerosis, HIV-related cognitive impairment, alcohol-related cognitive decline, and dementia.

Abigail B. Sivan, Ph.D.
obtained her doctorate from New York University. She is a consultant to Michigan Avenue Neuropsychologists and holds a faculty appointment at Northwestern University. Specialty areas of research and practice include developmental disorders, traumatic brain injury, neuropsychological assessment, child trauma and advocacy.

Babatunde G. Okuleye, M.D.
obtained his medical degree from the College of Medicine, University of Lagos, Nigeria, and completed residency training at the University of Chicago in general psychiatry and fellowship training in forensic psychiatry at the Isaac Ray Center in Chicago. He is a consultant to Michigan Avenue Neuropsychologists, and the President/Corporate psychiatrist at Health Professions, Ltd., a private correctional health care firm. Specialty areas of practice include independent medical evaluations and fitness for duty, correctional and forensic psychiatry.

S. Jan Brakel, J.D.
obtained his law degree from the University of Chicago Law School. He is CEO and Director of Training and Education at the Isaac Ray Forensic Group, and holds faculty appointments at DePaul University College of Law and at Rush Medical College. He is widely published in the field of mental health law and provides consultation and education to the clinicians of Michigan Avenue Neuropsychologists in legal cases.
